Complete the online registration forms here

All parents and guardians will be given login information for the Family Portal. This portal gives you access to important information and resources throughout the school year. Login information will be sent out by mid-July.

Login information will be sent to Parents by mid-July. This information will allow you to access and complete your Magnus Online Health Forms. These forms must be completed by July 22, 2022.

To complete these forms, please gather and be ready with the following information prior to the completion deadline:

• a recent physical exam (within 12 months)
• a full list of immunizations (please note that all age-eligible students will be required to have the COVID-19 vaccination prior to arrival)
• a copy of the student’s health insurance card that provides appropriate coverage. Domestic coverage cannot be emergency room coverage only or you will be required to take the school insurance. (if you are getting health insurance through the school, we will fill out this part of the form)
• a list of emergency contacts
• prescription medication orders from your child’s doctor
